Community Facility Use Requests
Click on the links below to download the editable facility use forms. After filling out the form, please email the building contact and attach the saved pdf. Requests will be considered first-come, first-served, with school activities taking priority. Use of facilities during school hours (typically 8:00 am – 4:30 pm) is not allowed. Please read Policy KG – Community Use of School Facilities and the accompanying administrative rule for details and procedures. Please note that charges may apply, and *insurance is required for all applicants.

* Insurance Requirements
All applicants must provide proof of general liability insurance coverage through a Certificate of Insurance. In addition to the general liability insurance coverage, the insurance coverage must include the automobile liability endorsement in the event of any accident involving a vehicle of the applicant group while on district grounds; automobile liability will be designated at $1,000,000 combined single limit per accident. This certificate must show that the insurance coverage will be in effect during the event date(s) and show minimum general liability coverage in the amounts of $2,000,000 per occurrence and $2,000,000 in aggregate liability coverage, with automobile liability at $1,000,000. The school district must be named as additionally insured on said policy, and a copy of the endorsement is to be included with the insurance certificate. Coverage cannot be canceled or reduced without 30 days written notice to the district. The superintendent may require additional insurance depending on the nature of the facility use.
