You will need to fill out a different request for each address you are requesting transcripts be sent to.

Replacement Diploma Certificates - If you need to replace a lost or damaged high school diploma certificate, please contact Miners Graduation Services. The links provided take you directly to the Miners Online Ordering platform. You may also contact Miners directly at info@minersgrad.com or by phone at 541-689-4588. Once you submit the required information to Miners, they will confirm your graduation stats and then mail the new certificate directly to you.
