A Word about School Attendance
Regular school attendance is so important for students' success.  At Siuslaw Middle School, we strive to make every child feel welcome
Did you know?
  • Students should miss no more than 9 days of school each year to stay engaged, successful and on track to graduation. 
  • Absences can be a sign that a student is losing interest in school, struggling with school work, dealing with a bully or facing some other potentially serious difficulty. 
  • By 6th grade, absenteeism is one of three signs that a student may drop out of high school. 
  • By 9th grade, regular and high attendance is a better predictor of graduation rates than 8th grade test scores. 
  • Missing 10 percent, or about 18 days, of the school year can drastically affect a student’s academic success. 
  • Students can be chronically absent even if they only miss a day or two every few weeks. 
  • Attendance is an important life skill that will help your child graduate from college and keep a job.

Sports registration has gone digital! - Please use the link to complete sports registration with FamilyID.  Electronic payment options are also available.  We have one district level access for both middle and high school students.
 

OSAA Physical Form - Students are required to have a physical examination for athletics at least every two years to be able to participate in school athletics.  The physical must be completed on the attached form.  The sports physical screens for specific health issues that may not be regularly covered in a general office visit.

 Transportation Information

 
Bus transportation forms are required for all students using bus services.
 

 
Nearly every place in Oregon that provides care for a child outside the home requires immunizations or an exemption to stay enrolled.

Students in the 7th grade will need a Tdap booster shot before the exclusion date: February 15th, 2017. Appointments may be scheduled with the Florence Peace Health Lab at 541-997-7134. Shots will not be administered at our local walk-in clinic. Students must have updated immunizations and/or exemptions turned in by Feb. 15th in order to continue attending school. You may contact Lane County Public Health at (541) 682 – 4041 with any questions.

 absences  Illnesses & Absences
If your student is ill, please call the office informing us of their illness so it can be marked as an Excused Absence.  Also, students should be "fever free" for 24 hours before returning to school. Your assistance in these matters is greatly appreciated!  
Please review our Student/Parent Handbook for more information regarding excused absences and attendance policies.

Siuslaw Middle School offers challenging academic courses, interesting electives, support programs for students and volunteer athletics.
Some of the programs include:
Full music program
Full PE program
Enrichment classes, aimed at academic support for struggling students
PBIS - Positive Behavior and Instructional Support
VIKS Team - Advisory Groups.
Two full computer labs
Library for students to learn new reading and technology skills
Athletics - staffed fully by volunteers
ASPIRE Program - Volunteer Program to enhance student success
